How to fill this out

Regular employee passenger: Employee / Non-Control / Business / Relationship = Self. Leave Default Host blank.
Control employee: choose Employee + Control only for officers, owners, or employees who meet the control/high-compensation tax definition.
Director: choose Director when the person is traveling in their director capacity; do not treat as a regular employee for 50% exclusion.
Independent contractor: choose Independent Contractor when they are not on payroll. If their trip is personal/non-business, usually assign a Default Host and Relationship = Independent Contractor.
Spouse/child/dependent: choose that classification, set Default Host to the employee they are connected to, and set Relationship to Host to match.
Non-related guest: choose Guest, set Default Host to the employee hosting/benefiting, Relationship = Guest / Other Passenger. These guests may still be imputed even when the 50% rule is met.
Default Purpose: Business for normal company travel. Use Personal only if this person normally rides for personal reasons unless overridden on the leg.
Security concern cap: check only for employees covered by a documented bona fide security concern / overall security program. Put the study or memo reference in Notes.
